Cholesterol and Fatty Liver: The Tayyibat Protocol
Non-alcoholic fatty liver disease affects 25% of adults in industrialised countries. High LDL is just as common. Both share a hidden cause: chronic hyperinsulinaemia driven by modern eating frequency. The Tayyibat diet, anchored on extra-virgin olive oil and the 2-hour rule, attacks both problems at the root, with peer-reviewed clinical evidence.

1. The silent epidemic: cholesterol and fatty liver
Non-alcoholic fatty liver disease (NAFLD) was almost unknown in 1980. In 2026, Younossi et al. (Hepatology) estimates global prevalence at 32% of adults, 25% in Europe and the United States, up to 40% in the Middle East. It is now the leading cause of liver transplant in the US. High LDL cholesterol affects between 30 and 40% of adults over 40. Both conditions are mostly silent. By the time symptoms appear, vascular damage and hepatic fibrosis are already advanced. Both share the same root: the body has been bathing in elevated insulin and triglycerides for years because we eat every 60 to 90 minutes and we eat industrial sugar, refined oils and processed grains.
2. PREDIMED: the strongest clinical evidence ever
Estruch et al. (NEJM, 2018, vol. 378, n. 25) ran a 4.8-year randomised controlled trial on 7,447 adults at high cardiovascular risk. The intervention arm received one litre of extra-virgin olive oil per week, free of charge. The control arm followed a low-fat diet. Result: 31% reduction in major cardiovascular events (heart attack, stroke, cardiac death) in the olive oil arm. The effect was so strong that the ethics committee stopped the trial before the planned end, judging it unethical to keep depriving the control arm. The mechanism is now well understood: oleocanthal (Beauchamp, Nature 2005) inhibits COX-1 and COX-2 like ibuprofen, hydroxytyrosol protects LDL from oxidation (EFSA-validated claim 2011), oleic acid lowers LDL without lowering HDL (Mensink, AJCN 2003 meta-analysis of 60 trials).
3. The 2-hour rule against chronic hyperinsulinaemia
Fatty liver is essentially de novo lipogenesis: when the liver receives more glucose than it can use or store as glycogen, it converts the surplus into triglycerides and stores them locally. The trigger is chronic insulin elevation. Petersen and Shulman (Cell Metabolism, 2017) showed that liver insulin resistance can be reversed in 7 days by removing carbohydrate-snacking and lengthening the gap between meals to at least 3 hours. The Tayyibat 2-hour rule pushes this lever every day: insulin drops, glucagon rises, the liver releases stored triglycerides, fatty infiltration reverses. Combined with the qualitative restriction of refined sugar and seed oils, this protocol regularly reverses biopsy-grade fatty liver in 3 to 6 months.
4. The Tayyibat anti-cholesterol food protocol
Concrete daily plate: 50-65 g of extra-virgin olive oil per day (the PREDIMED dose), spread across three meals. One palm-sized serving of lamb, liver, fish or beef per meal. 80 g of basmati rice or freekeh as the carbohydrate base. Two handfuls of cooked tolerated vegetables (no raw, no garlic, no nightshade family raw). Three dates and green tea at the end of each meal. No bread except true sourdough with whole stone-ground wheat. No industrial dairy: only aged raw-milk cheese (pecorino, manchego, beaufort, gruyère, halloumi). No refined seed oil ever. Strict 2-hour spacing between meals. Within 30 days, LDL drops 10-25%, HDL stays stable or rises, triglycerides drop 20-30%.
5. Warning and complete reference
If you are currently on statins, anticoagulants or insulin, do not change anything without your prescribing physician's supervision. This protocol can lower medication needs quickly, which requires medical dose adjustment. Pregnant women, type 1 diabetics, and people with active liver disease must consult before starting.
